1. Self-Managed:

Self-Managed Participants can use the invoice provided to claim directly from the NDIS themselves, (if funds are available in the appropriate support category).

Once you have placed your order, please email admin@theminimunchkins.com.au and we will provide you with a specific tax invoice to submit for your claim. Please include the NDIS number in the email.

2. Plan-managed:

If you are Plan Managed, and would like us to invoice your plan manager directly, please fill out the form below:

We will process the order for you and invoice the Plan Manager directly. Once funds are received from the Plan Manager, we will then dispatch goods directly to the participant.
